How to submit a request

Requests must be in writing and the records you are seeking must be reasonably described. DHS FOIA regulations require that you describe the records you are seeking with as much information as possible to ensure that our search can locate them with a reasonable amount of effort (6 C.F.R. Part 5, Sec. 5.3(b)). Please make sure your request includes: Name, Contact information (street address or email address), Brief description of records sought. If you are seeking records that contain personal information about yourself, provide a perjury statement and signature or notary. If you are seeking records that include personal information of a third party, you may receive greater access by providing a third-party release statement. If the subject of your request is deceased, affirmation of his/her death, such as a copy of a death certificate or an obituary column, is required.
