How to submit a request
Due to the current federal funding hiatus since February 17, 2026, there will be a delay in FOIA processing within most FOIA offices within the Department.
What you'll need
- If you are requesting records about yourself, submit a signed Certification of Identity form, or a perjury statement and signature or notary.
- If you are requesting records about someone other than yourself (third-party), submit a signed Affirmation/Declaration form and/or a signed Form G-639 with your request.
- If the person you are requesting records for is a minor, include proof of guardianship (court documents appointing guardianship, birth certificate, or Power of Attorney).
- If the person you are requesting records about is deceased, submit proof of death (death certificate, news article, or obituary column).
- If you are an attorney, it is also recommended that you submit a signed Form G-28.
